This intake form covers one service request at a time. If you need multiple services (for example, Cerakote and Stippling on the same item, or services on more than one firearm), please complete and submit this form once for each service.

Submitting separately helps us review, quote, and schedule each request accurately. All of your submissions will be linked to your contact information on our end.

Lead times vary based on service type, complexity, current workload, and material availability. 

Any timeframes discussed or published are estimates only and are not guaranteed until your item has been reviewed and approved.

Items are received in working condition unless otherwise noted. Pre-existing defects, wear, or internal issues may not be visible prior to disassembly or refinishing and are not the responsibility of Custom Defense Firearms LLC.

Refinishing, coating, laser marking, or modification may void manufacturer warranties. It is the customer’s responsibility to verify warranty implications prior to service.

Cerakote, KG GunKote, and similar finishes are durable but not indestructible and are subject to normal wear, holster wear, handling, and environmental exposure. Multi-color or layered finishes are cosmetic in nature and are not warrantied.

Polymer, plastic, hybrid materials, optics, electronics, and accessories are processed at the owner’s risk due to heat, tolerances, and material variability.

Tight-tolerance or hand-fitted parts may require post-coating adjustment. Minor fitting, wear, or finish contact in these areas may occur.

Custom Defense Firearms LLC is not responsible for damage caused by shipping, improper installation, misuse, storage conditions, or work performed by third parties after service.

Items left unclaimed for extended periods may be considered abandoned in accordance with shop policy.

By signing below, you authorize Custom Defense Firearms LLC to perform the requested services and agree to release, indemnify, and hold harmless Custom Defense Firearms LLC and its employees from claims or damages arising from the work performed, except in cases of proven gross negligence.
